Tshepo was leaning on the machine when Thabo was demonstrating. Tshepo's unbuttoned shirt got caught by the high speed rotating chuck, pulled his hand and chopped it out. He screamed for help while bleeding and blood was oozing from his injured hand. One learner ran to the classroom next door to ask for assistance.

On Monday, 13th March 2021, Mr. Van Duran, a lecturer at Moline High School's Fitting and Turning workshop, was demonstrating to his learners how to operate a lathe machine. Mr. Van Duran hastily left the workshop without providing any instructions to his students about what to do with the machine in his absence.

One of the students in the workshop, Thabo, whose father owned a mini workshop, took it upon himself to try and demonstrate to his fellow classmates how to operate the machine in Mr. Van Duran's absence. Tshepo, another learner in the workshop, was standing too close to the machine and leaning on it while Thabo was demonstrating. Unfortunately, Tshepo's unbuttoned shirt got caught by the high-speed rotating chuck of the lathe machine, pulling his hand and severely injuring it.

Tshepo screamed out in pain as his hand was chopped off by the powerful machine, causing blood to rapidly ooze from his wound. In a state of panic and distress, one of the learners quickly ran to the classroom next door to seek help for Tshepo.
