During his last years, Fred McNeill suffered from severe neurological issues and was eventually diagnosed with chronic traumatic encephalopathy (CTE), a degenerative brain disease found in individuals with a history of repetitive brain trauma. He struggled with memory loss, cognitive decline, and motor problems, and his condition continued to deteriorate. McNeill passed away in November 2015 at the age of 63 due to complications from ALS, which was believed to be related to his CTE. His struggles with the devastating effects of brain injuries and the toll they took on his health and well-being were chronicled in the documentary "League of Denial: The NFL's Concussion Crisis."
