ok so to summarise the task i have to consider scenario where the patient comes to you, you gave her all the medications that normally would work 100% on all patients, yet here it has no effect. Its obvious the patient didn't stick to the treatment you recommended. And the question is: how would you deal with a patient in such a situation? i don't know what to say to patients then, how to respond? any ideas? :)

I'd ask the patient his/her reasons for not following the recommended treatment. Listen to what the patient says  and you may get an idea about how to help.
When my mother, who had Alzheimers, had just moved to a senior citizens home, her opthalmologist saw that she was not using her eyedrops. He told me to get the nurse to administer the drops.
