How do i write an application for a day's leave from school?

I would use a memo format -- http://owl.english.purdue.edu/handouts/pw/p_memo.html -- and make the text of the memo short and to the point.

"I am requesting leave on Tuesday, February 27, 2007, because of ___________________________."

In the blank, put your reason in simplest terms: "a doctor's appointment" or "an interview at Bennington College" or whatever.
