I strongly disagree with both of your answers for 5. If this were a man, I don't think you'd hesitate to address him as Mr. Peters. The equivalent for a woman is Ms. Peters. It assumes her marital status is also irrelevant in this situation. Madame Vice President is way too formal and Caroline is way too informal.
