I think you're supposed to write a paragraph explaining how your school can affect your job and your future. And the part, "Include the subject areas that are most important to your career goals, and what you might realistically do to improve your grades in those subject" probably means choose subjects that you will use for your future job and what you'd do to increase your grade in them. For example, if you want to be a scientist and you have an F in science class, how would that affect your future and how would you change the grade?

Okay so, imagine if you were in medical school trying to become a nurse (RN). During that time at school you have to go through multiple classes and tests. And to move on to the next course you have to pass the test with a certain grade. If you don't achieve that grade you don't move on, and once you fail enough classes you can get kicked out of that program, so you wouldn't become a nurse. And so to get those high grades on tests you need to study a lot and complete all the classwork.
