Can you pls give me a clue what is the answer to the fourth one.

Patriarchs were at the top of the hierarchy in the Eastern Church, but bishops headed the Church in the early Roman Church. I wasn't able to find when archbishops became prominent.

I'm confused because my book said that the patriarchs were at the top of the early Christian church hierarchy.

Then -- use the answer you found in the book. The early church hierarchy could include two or three hundred years.
