Look carefully at the directions. Do you see "using Direct & Indirect Object PRONOUNS?" You have not done that. You have used the nouns.

1. Leí el libro a Halena. = Se lo leí a Helena. The Indirect Object Pronoun is "le" which becomes "se" when followed by "lo." The rule is that either "le" or "les" followed by a Direct Object Pronoun beginning with "l" = lo, la, los, las, becomes "se." Then because you don't know whether "se" refers to "le" or "les" the "a Helena" clarifies it.

2. Quiero leer el libro a Helena. = either (because with an infinitive, you have the choice of putting all object pronouns either in front of the conjugated verb (a) or after the infinitive (b), attached to it, being careful to put any necessary accent mark.
(a) Se lo quiero leer a Helena.
(b) Quiero leérselo a Helena.
