Which sentence is grammatical? Are both OK?

#3 is grammatical. What about #4? In Sentence 4, 'where' is missing. Is #4 grammatical?
#1 is correct IF there are other Toms and you're referring to the one "who lives there."
#2 is preferable because usually, when you use someone's name, you are referring only to that one person.
#3 and #4 are correct. Either way works fine.
