How would you put this in an equation in slope intercept form? what would it look like?

Slope 1/2 ; x-intercept 4

for a given point (x1, y1)

and a slope of m
we can find the equation by saying:
y - y1 = m(x - x1)

since an x-intercept of 4 simply means we have the point (4,0)
we get

y-0 = (1/2)(x-4
or
y = (1/2)(x-4)
juggle this any way you need
e.g
2y = x-4
or
x-2y = 4
or
y = (1/2)x - 2
