Myelin visited my office yesterday complaining of numbness in his right leg and face. He has also been experiencing muscle weakness and mild depression. I noticed a delay in his visual response in both eyes. I have referred Myelin to your facility to have an MRI of the brain and spine. Please fax the results to me as soon as possible.






I’m sending little Jimmy over for a stat spinal tap. He has had a fever for a couple of days, which increased to 102 degrees today. Since yesterday he’s been complaining of a headache and stiff neck. His mother said last week he had gone swimming in the pond by their barn and started feeling ill a few days later. Fax results ASAP.

    1. What nervous system disease is Myelin suffering from?
    The disease is Multiple Sclerosis

    2. What could have caused Myelin's disease?
    It’s an autoimmune disease and exact causes are still unknown.

    3. What structure(s) and function(s) of the nervous system are affected by this disease?
    The neurons of nervous system involving the brain and spinal cord are affected. Actually a special coating called myelin covers axons of these neurons, which improves the conduction of electric impulses between them. In multiple sclerosis, this myelin sheath is lost or inflamed or destroyed leading to formation of a scar over the axons.
    This leads to less effective communication between neurons and hence symptoms of multiple sclerosis, like vision is affected.


    4. What can be done to treat this disease?
    There is no exact cure yet and medications are mainly provided to control the progress of disease. Special chemicals called beta interferons are used to regulate the immune system like betaseron. Also glatiramer acetate, a synthetic form similar to myelin is used.
    Mitoxantrone, an anticancer drug is used by intra-venous route, to weaken the immune system, as this helps in decreasing the symptoms of Multiple sclerosis.

    It is Meningitis i.e. inflammation of meningitis of brain.

    2. What could have caused Jimmy's disease?
    It is commonly viral in origin and called viral meningitis (less dangerous though). It is caused by viruses like enteroviruses, varicella zoater virus, mumps virus, etc. Bacterial meningitis is rare but can be deadly and caused by Neisseria meningitis, Streptococcus pneumonia, Staphylococci.




    3. What structures and functions of the nervous system are affected by this disease?
    The meningitis i.e. the 3 membranes that line the brain and spinal cord are affected. They are Dura mater, arachnoid and pia mater. Due to inflammation of these membranes, there is fever, neck stiffness, confusion, nausea and vomiting. Children may be irritable, may not eat well.

    4. What can be done to treat this disease?
    Bacterial meningitis requires administration of intravenous antibiotics. The type of antibiotic used depends on the causing organism. They should be started earlier in the course of disease to prevent mortality. Patients also require symptomatic treatment like Intravenous fluids and also rest and should be kept under observation. Management is same for viral meningitis except that it doesn’t require antibiotics.
