As I indicated in one of your later posts, words ending in "-ly" tend to be adverbs.
To determine whether the word in parentheses is an adverb or an adjective, you can look at how it is modifying the noun or verb in the sentence.

In the second example, "fairly" is modifying the adjective "severe," which describes the intensity or extent of the abdominal pain. Therefore, "fairly" is an adverb, and "severe" is an adjective.
In the third example, "unstable" is describing the noun "angina," indicating the condition of the angina. Therefore, "unstable" is an adjective.
