Maybe these examples will help. I believe this is how to do them.
Expanded form of 40.035 is...
40 + .0 + .03 + .005
Word form of 40.035 is...
4 tens + 0 ones + 0 tenths + 3 hundredths + 5 thousandths
Fraction of 40.035 is...
8007/200 (I used a graphing calculator... don't know how to do by hand) 
the fraction form of 40.035 is
40035/1000 Notice the denominator came from the smallest numeral in the fraction (in this case thousandths).
Of course, you reduce it.
