If the water is not flowing very fast the water condenser jacket will not fill with water, the vapor in the inner tube will not be cooled to condense it, and the vapors will exit the inner tube and flow into the room. I know because I made that mistake one time. By the time I figured out what was wrong I had lost about half or more of the product I was to prepare. The other half of the story is that you get razed a good bit but you won't make the mistake twice.
