Editing grammar.

Review of the sleep questionnaire indicates that the patient had a Trend Oximetry done 2 years ago, and was placed on 9 cm of sea pap. The patient notes that he has not seen much change with the use of it. His Father has a history of the same. He still continues to have day-time fatigue and sleepiness and some shortness of breath. There is also a history of seizure disorder diagnosed in the Fall of 1997.
