Homework Help: Chemistry

For a molecule of fluorous acid, the atoms are arranged as HOFO.

What is the formal charge on each of the atoms? Enter the formal charges in the same order as the atoms are listed.

(I searched everything online to find the shape of HOFO but could not find it--(they are wrong?!?))
(Could you please give me the correct Lewis structure?)(I know how to calculate formal charges: but have gotten wrong answers these past few hours) THANKS.
