The formal charge is 0 on H and 0 on the O of HO. F is +1 and the right most O is -1.
If I draw the Lewis structure for HOFO, I count four regions of high electron density which translates to tetrahedral electronic shape; however, I can't find anything on the net or in my texts that will confirm that.
