Hi, I have to write an opinionated article on the safety problems in space. I have to wrtie the issues that occur during a lift-off which are ignored I have been unable to find a website that tells me issues that occur but are ignored or thought that because they have back up things will be fine.All it is giving me are the issues which can happen, etc.

If you are not finding assistance online, I, for one, am not surprised. To my way of thinking, no one will want to ADMIT to ignoring safety issues. The "track record" on space flights is not perfect, but what guilty party is willing to admit that?
Why not take what you DO find, saying something such as: IF these situations are ignored. . . . . and then just propose some problems that might happen? Unfortunately you will have to work with what you find, so just try to make it work for you?
