Make sure you answer question in a complete paragraph that includes a introductory sentence at least four sentences of explanation, and a concluding sentence. check your spelling grammar, punctuation and sentence structure. Provide a clear organization include words like first, however, on the other hand, and so on consequently since, next and when.

My first question. Should corrections be date and time stamped?

Information may sometimes be incorrectly entered into a medical record. When this happens, the error must be corrected. The person who made the error should also make the correction, however, it shouldn't appear that the records are being altered to cover up a mistake in diagnosis or treatment. Correction rules apply for both written and electronic records. When a medical record exists in eletronic form, errors are usually corrected in the form of an addendum. The original record remains intact, and isn't actually altered. The proper method to correct a written record is to draw a single line through the entry and write "error" next to it. In conclusion, additions to records should be time and date stamped, and the person making the additions should be identified in the record.
