Not sure about some grammar

Un moment mémorable de mon enfance était avec mon grand-père et moi(do i need "et moi"?). J'étais en troisième année. J'avais quatre ans, de plus, il était sur mon anniversaire. Comme je pleurais, mon grand-père m'a amenée au parc to play (is it jouer?).
Le parc était près de ma vieille (translation: my old house, because I moved and now I live elsewhere. Is this adj correct?) maison et nous avions l'y habitude d'aller toujours. Je me suis rappellée que mon favori était la balançoire (english = swings, so do I write les balançoires?). D'habitude mon grand-père me pousserait sur la balançoire(eux? trying to use pronouns:) ). J'ai pensé que c'était très amusant et agréable.

    et moi? = no, drop that and put the period after "grand-père."

    en troisième année. = dans la troisième année. (I don't understand this: if you were in the 3rd year, how come you were 4 years old?)

    il était sur mon = il était mon, etc.

    amenée au parc to play = looking at the past participle (amenée) you must be a girl...au parc pour jouer. = to the park to play

    avions l'y habitude = either "avions l'habitude....OR if y ou wanted to say "we had the habit there (referring to the house) = y avions l'habitude

    mon favori était = just saying my favorite seems insufficient = my favorite what? = ma chose favorite c'était...

    was the swing or was the swings? If you want the swings = les balançoires (seesaws) vs. les escarpolettes (swings)

    me pousserait sur la balançoire(eux? = m pousserait sur elles (escarpolettes)

    J'ai pensé = I thought (meaning the thought went through my head) vs J'ai cru (I thought/believed)

    P.S. Simply "sh" does not tell me if you are male or female and there were 2 past participles that needed to know!

    since swings are les escarpolettes, does it turn to -> c'etaient les escarpolettes?

    ohhh I forgot to agree to the adjectives,
    so its amusante and agreablee
