What is the difference between a legislative bond measure, a legislative constitutional amendment, and an initiative constitutional amendment? How many of each are on the ballot?

What ballot are you referring to?

In the federal constitution, there is no such thing as an initiative. Constitutional amendments are initiated by Congress or by a Constitutional Convention. (The latter has never happened since the Bill of Rights)

It sounds like you are referring to State government bond issues and initiatives.
