Active voice is when the subject is doing the action of the verb.
* Jack threw the baseball.
* I made myself a sandwich.
Passive voice is when something/someone else is doing the action, usually preceded by the word "by."
* The baseball was thrown by Jack.
* The sandwich was made by me.
In the sentence you were given, the main verb is "was set up" -- it's in passive form because it was the supervisor that did the setting up. How can you reword that so that "supervisor" is the subject of the sentence and the verb is active?
The supervisor set up the new recycling program in our office.
