#4 is right, yes.
Normally, a Church affiliated hospital is a non-profit facility.
A government example would be a veterans hospital, and most are non-profit.
State hospitals are often teaching hospitals, and most are non-profit.
Depending on the type of your listed hospital, would help you choose a catagory.
I too think number 4 would be correct.
