Find an equation for the line that contains each pair of points. If the line is not vertical. Write the equation in slope-intercept form.

X(4,5,0) and Y(0,4,5)

Your two points should be defined by two numbers, not three. Should some of your commas be decimal points? Calling the points X and Y and asking for an equation in the form y = mx + b is a bit confusing.
