I suspect you're referring to whether the patient pays for his/her own medical care which is privately paid care. People with private medical care usually buy insurance (often with the help of their employers) that pays most of the costs of their medical care.
By social medical care, you probably mean medical care paid by the government from increased taxes from all citizens and businesses. For the patient, social medical insurance pays all of the medical bills.
